IRCCO provides a comprehensive suite of services designed to support our ACO participants in delivering high-quality, coordinated care. These services include:
- Data Analytics & Reporting: We aggregate and analyze clinical and claims data to help providers identify high-risk patients, track performance metrics, and meet CMS and other payer reporting requirements.
- Care Coordination Support: While clinical care is delivered by our member organizations, IRCCO offers training, workflows, and tools to enhance care transitions, chronic disease management, and patient engagement.
- Compliance & Regulatory Guidance: We ensure that all ACO activities align with CMS regulations, including HIPAA, Stark Law, and MSSP requirements.
- Performance-Based Incentive Distribution: IRCCO administers shared savings distributions based on participant performance in key patient-engagement areas such as Annual Wellness Visits (AWVs) and care management for polychronic and high-risk patients.
- Education & Training: We provide ongoing education for our members on value-based care strategies, coding accuracy, quality improvement, and community resource engagement.
IRCCO’s role is to enable and support—not to replace – the clinical expertise of our member providers. Together, we work to improve outcomes for rural Medicare beneficiaries across Illinois.
